Patients With Insurance
We accept auto accidents, workers compensation, personal injury and most health insurance plans in our office. We will verify and explain your insurance benefits to you at no charge. You can give us your insurance information over the phone, fax a copy of both sides of your insurance card to our office, or bring your insurance card by our office so we can make a copy for you. In order to verify your insurance we will also need your date of birth.
Veterans Community Care Program
We are a Provider with the VA Community Care Program (formerly the Veterans Choice Program). This program covers Chiropractic, Acupuncture and Myofascial Release Therapy with no deductible and no co-payments. We are covered under the Chiropractic Authorization, Acupuncture Authorization, and Pain Management Authorization. See our Veterans Community Care Program page for more information.
Patients Without Insurance
For patients without insurance, we have discounts for payment at the time of service. Our time of service prices are reasonable, and since we get patients out of pain in as few treatments as possible, patients can afford care in our office without insurance coverage. Call us for more information on treating in our office without insurance coverage.
Massage Therapy / Myofascial Release Therapy First Visit
Your first 60-minute massage therapy/myofascial release therapy session in our office is only $60. (You do not have to be a Chiropractic patient in our office to see our therapists, and they will not pressure you to become a Chiropractic patient.)
Online Forms
We utilize Electronic Medical Records (EMR) by Medicfusion. You will need a secure Access Number and valid email address to complete your new patient forms online. Please contact our office at 480-377-1226 for your secure Access Number.
Once you have your secure Access Number click here to register and log in. If you are already registered, click here to log in. If you would would prefer not to complete your new patient forms online, you can complete a paper version of the forms on your first visit our office.
