 First Visit InformationOut Of Office Labor Day WeekDr. Brown will be out of the office the week of Labor Day, from Monday, September 6th through Friday, September 10th. While he is gone, Dr. Brown recommends the services of Dr. Mark Strom (Naturopathic Medical Doctor, www.drmarkstrom.com) at 480-219-2351, and Dr. James Garret (Doctor of Chiropractic, www.optimalhealthcenter.net) at 480-730-9636. We will return to the office on Monday, September 13th at 9:00am. Massage Therapy with Rachel will be available by appointment on Thursday, September 9th and Friday, September 10th. Personal Training with Garret will be available by appointment on Friday, September 10th. Office Location Our office is located in Tempe, Arizona, just outside of Phoenix and within an easy drive of Ahwatukee, Scottsdale, Mesa, Chandler and Gilbert. We are in the Crossroads Professional Plaza at the southwest corner of Price and Baseline in Tempe, next to the Tempe 24-Hour Fitness health club. Call us at 480-377-1226. Office Hours Monday-Wednesday-Friday 9:00am-1:00pm, 2:00pm-6:00pm Tuesday-Thursday 2:00pm-6:00pm Patients With Insurance We will verify your insurance benefits for you at no charge. You can give us your insurance information over the phone, fax a copy of both sides of your insurance card to our office, or bring your insurance card by our office so we can make a copy for you. In order to verify your insurance we will also need your date of birth. Patients Without Insurance For patients without insurance coverage, we have discounts for payment at the time of service (TOS). Our TOS prices are very reasonable and patients can afford care in our office without insurance coverage. Call us for more information on treating in our office without insurance coverage. Online FormsWe utilize Electronic Medical Records (EMR) by Medicfusion. You will need a secure PIN # and valid email address to complete your new patient forms online. Please contact our office at 480-377-1226 for your secure PIN #. Once you have your secure PIN # click here to register and log-in. If you are already registered click here to log-in. If you would prefer not to complete your new patient forms online, you can complete a paper version of the forms on your first visit our office.
